Direct Deposit Information and Form

To begin receiving your check through direct deposit:

1. Please print the form from this link: Direct Deposit Form
2. Fill it out and return it to our office with a voided check.
3. If you are depositing your paychecks into your saving account, please return form with a Savings Deposit Slip.
4. You may either mail the form to us at:

   You may drop it by our office:
   324 B Caldwell Blvd. Nampa, ID 83651

Please note:

• Because we send the account numbers through to ensure accuracy, you will receive your upcoming check as a "regular" paycheck. The following payroll check will then be deposited directly into your account and a check stub will be sent to you for your records. If there is a snag, our Payroll Department will contact you.
• Because AAA Home Care has a Saturday pay date, your money should be available to you on that following Monday morning. (If Monday is a holiday, then the deposit will be on Tuesday morning). If you ever find that it is not in your account, please contact our office and your local bank.
• AAA Home Care pays bi-weekly. Please refer to your pay calendar for payroll dates.
